Elite Health Care got its start in early 2016 by a team of dedicated individuals with a passion for helping adults with disabilities. The goal? To create an agency and provide services where participants felt empowered, respected, and inspired to grow. From that small beginning, Elite Health Care has grown to an established, successful agency helping participants strive to become their best selves. Elite Health Care now includes a dynamic, experienced administration team and an expanding group of devoted, exceptional direct care staff.

Isabella Perry
Program Director
With a passion for empowering individuals with developmental disabilities, Isabella is committed to fostering independence, dignity, and quality of life for every individual served. Isabella received her Bachelor's degree in Social Work from Idaho State University and as Program Director, Isabella oversees the daily operations, staff development, and person-centered care that ensure each resident thrives in a supportive and respectful environment.

Terry Hodgson
Finance Specialist
With a strong foundation in caregiving and years of hands-on experience supporting individuals with disabilities, Terry brings a unique, person-centered perspective to their role as Finance Specialist. Blending compassion with precision, he manages financial operations, payroll, payee services, and budgeting processes to ensure the highest quality of care and accountability. Terry is dedicated to supporting both the organization and the individuals it serves through ethical and efficient practices.
Keith is the man behind the vision and origination of Elite Health Care. He has a strong passion for helping individuals with disabilities and believes in the potential of all participants to live independent and fulfilled lives. As Administrator, he directs and leads the company in providing meaningful services to participants.

With years of experience supporting individuals with disabilities and caring for loved ones, Kimberley brings deep compassion and practical insight to her role as Regional Training and Relations Specialist. Her background in horse training has strengthened her skills in patience, communication, and growth-focused guidance—qualities she now uses to mentor staff and empower others. Kimberley is dedicated to fostering strong relationships and creating environments where both teams and individuals can thrive.
Originally from California and now bringing fresh energy to Idaho, Erin specializes in recruiting and marketing with a mission-driven focus. With a strong track record of helping companies grow while maintaining high-quality services, she is passionate about connecting great people with meaningful work. Erin is dedicated to ensuring individuals with disabilities receive the highest standard of care through strategic outreach and talent development.

Activities Director
As an Activities Director and passionate dance instructor, Megan brings creativity, energy, and joy to every day. With a heart for working with individuals with disabilities, she designs engaging, inclusive programs that promote movement, connection, and self-expression. Megan believes in the power of fun and creativity to uplift and empower every individual.

Jordan Lazalde
Activities Coordinator
Jordan is a dedicated Activities Coordinator with a passion for creating fun, meaningful experiences for individuals with disabilities. Known for his patience and excellent listening skills, Jordan focuses on fostering inclusion, joy, and personal connection through every activity. He is committed to making each day engaging, supportive, and full of opportunity.

Reyna Borjourez
Office Manager
With a Bachelor's degree in Business and personal experience caring for loved ones with disabilities, Reyna brings both heart and expertise to her role as Office Manager. Passionate about supporting individuals with disabilities, sh is committed to ensuring smooth, efficient operations that help deliver compassionate, high-quality care in a residential habilitation setting.

Lauren Carroll
QIDP
Lauren brings a well-rounded background in healthcare and a strong academic foundation, holding a Master’s Degree in Mental Health Wellness from Grand Canyon University and a Bachelor’s Degree in Psychology from the University of North Carolina. With diverse experience across healthcare settings, she is dedicated to supporting individuals with intellectual and developmental disabilities through compassionate, person-centered care and advocacy.

Berenice Lazalde
QIDP
Berenice holds a Bachelor's degree in Criminal Justice from Weber State University and brings a diverse background in healthcare, banking, and social services. With experience working in the ER and assisting individuals with disabilities and payee services at a bank, she is passionate about helping people navigate challenges and improve their quality of life. Berenice is committed to making a positive impact through compassionate, reliable support in her role as a QIDP with her participants

John Bowen III
QIDP
With over 40 years of military service, a Bachelor's in Psychology, and current pursuit of a Master's in Behavioral Analysis, John brings discipline, insight, and deep compassion to their role. Their years of experience working with individuals with disabilities reflect a strong commitment to understanding behavior and promoting growth. John combines real-world leadership with evidence-based strategies to support meaningful, person-centered outcomes in his role as a QIDP.
Our Staff
Our Direct Care Staff provide individually tailored services and supports in participants' homes and in the community. Each direct care staff is First Aid/CPR certified and if needed, medication administration certified. Each staff is oriented and trained on the specific participant's needs and abilities. Direct care staff also receive training concerning daily living skills, emergency situations, data tracking and collection, behavior management and de-escalation, and participant-specific goals.
